Given the weird pricing in stores in the US with not showing the full price including VAT in the shelves, I bet this scenario comes up a lot

That must be the most confusing part of it. Making it feel like the cashier is picking a random number if you are shopping in a new placeWell, yes and no. It's never that extreme but you shouldn't be surprised if $16.99 × 2 = $37. What I find weird is that each state (and sometimes each city) has a different rate.
When I go shopping, I just know some tax will be added. I don't even know what the rate is where I live. I do know, however, that food and clothing are exempt. Here, at least.
